Please complete and submit this registration form to me as soon as possible.  I will phone or e-mail you, to arrange payment. - 10 Week Session Before Christmas (Session 1) - ($70.00)
- 10 Week Session After Christmas (Session 2)   - ($70.00)
- 20 Weeks Both Sessions (Session 1 & 2)        - ($140.00)

By submitting this form, I give my approval to my child's full participation at the Power Skating Skills School. I further agree to hold harmless the Power Skating Skills School, it's staff or the Arena, from any and all claims for injury or loss, however caused.

You Must be a registered player with Lawfield Minor Hockey to enroll in this class. No Exceptions.

Note: - During the first week or two, players will be assessed by PSSS coaches to see that they are booked on an appropriate time slot. Players will be moved to the time slot best suited to their abilities so that all players get the maximum benefit of this program .... No Exceptions.
